Personal Care Services

Level 0 – Basic Assistance

The resident is capable of performing his/her Activities of Daily Living.

Services provided:

• 24-hour Nursing Service
• Assist with ordering medical supplies/DME
• Schedule physician appointments as needed
• Assist with management and organization of information to be taken to physician visits
• Arrange and schedule lab tests, diagnostics, and x-rays
• Store and administer medications
• Package fee — no additional charge

Level 1– Supervision

The resident requires cueing and supervision for performing Activities of Daily Living.

Services provided:

• All services identified in Level 0
• Reminders and supervision for Activities of Daily
• Living, including mobility, hygiene, toileting, and individualized medical needs
• Package fee is $200 per month

Level 2 – Limited Assistance

The resident requires hands-on assistance with their Activities of Daily Living.

Services provided:

• All services identified in Level 0
• Reminders as needed, identified in Level 1
• Hands on assistance for Activities of Daily Living, including mobility, hygiene, toileting and individualized medical needs
• Package fee is $400 per month

Level 3 – Extensive Assistance

The resident requires maximum assistance with their Activities of Daily Living.

Services provided:

• All services identified in Level 0
• Reminders as needed, identified in Level 1
• Extensive assistance to complete Activities of Daily Living, including mobility, hygiene, toileting and individualized medical needs.
• Package fee is $600 per month
