Our Hospice Services
Family Hospice brings care to you, allowing you to be where you are most comfortable.

Threshold Choir
Family Hospice works with the Great Lakes Erie Chapter of Threshold Choir. Founded by Kate Munger in 2000, the group's mission is to sing a cappella in small groups at the bedsides of people at the thresholds of life. Groups of two to four singers visit bedsides by invitation to provide comfort to hospice patients.

Choosing Family Hospice for Hospice Care
Did you know the law guarantees you the right to choose your hospice provider? Be sure to share your hospice choice with your physician and other health care providers. Ask for Family Hospice by name.
When you choose Family Hospice, our promise to you and your family is to help:

Eligibility Requirements
A licensed physician must first authorize hospice care and agree to participate in planning of care. Support person(s) are identified and agree to participate in planning of care.
To receive care from Family Hospice, patients must live within Erie County and:
- Have a life expectancy of six months or less.
- Prefer comfort measures instead of aggressive treatment.
- Desire hospice service and be aware of diagnosis and prognosis.
