The independent core service package is provided to you if you do not require assistance with ADLs and includes the following assisted living services:
The level one enhanced core service package includes all of the assisted living services provided under the Independent Core Service Package (as described above) and shall be provided to you if you require a moderate level of assistance with ADLs for an undefined recovery period, including but not limited to bathing, dressing, and personal hygiene.
The level two enhanced core service package includes all of the assisted living services provided under the Independent Core Service Package (as described above) and shall be provided to you if you require an extensive level of assistance with ADLs for an undefined recovery period, including but not limited to incontinence care, assistance with ambulation, transfers requiring a mechanical lift device, cueing and redirection, and scheduled or increased ADL and medical needs associated with aging in place.
A $500 non-refundable community fee is required upon admission.
