You must meet an income guideline to receive free services. If you qualify, simply call the Living-at-Home Program office to schedule a home visit.
The First Step
A care coordinator from the Living-at-Home Program will visit you at home and will ask questions about your health history, your medications, and any functional limitations you may have. Your answers will help the care coordinator identify your needs and determine the kind of services that can help you continue to live independently. If you qualify to receive services, a registered nurse and a social worker will complete an assessment with you.
