Pain Assessment: The CHOIR Survey

If you have chronic pain, we have a tool to help you answer the question, "Where exactly does it hurt?"

Stanford Health has created an online Collaborative Health Outcomes Information Registry (CHOIR) self-report body map to help you describe:

  • Where you feel pain in your body.
  • How intense the pain is.

It also gives UPMC Pain Management staff details about your general and mental health, daily life, and sleep.

We have you complete a CHOIR assessment before each visit. You may do this at home or when you arrive at the clinic.

CHOIR helps us learn how you experience pain throughout your treatment.

Your doctor will use the results to help decide which treatments will work best for your pain.
