A residential facility for people with an illness or disability who need assistance with their daily living activities, such as bathing, dressing and using the toilet. Skilled nursing facilities provide 24-hour care and short-term rehabilitative stays for patients recovering from an injury or illness. Some facilities have a separate unit for residents with Alzheimer's or memory loss.
A facility that provides hospital-level care for patients who require hospitalization for a period of time that is longer than usual.
Equipment ordered by a doctor for use in a patient's home. Examples include walkers, crutches, wheelchairs, hospital beds and oxygen.
There are many community agencies that may be able to provide assistance for you or your loved one after you return home. Make sure your discharge planner provides you with information about these resources. These agencies may be able to provide services like transportation, homemaker services or respite care.
Healthcare provided by medical professionals in a patient's home to maintain or restore health. This may include nursing care, therapies and limited assistance with personal care. Home health care involves brief, intermittent visits with the goal of helping patients and families become independent with care.
Hospice provides care for terminally ill people and their families. Hospice care can be provided at home or in a facility. This type of care focuses on improving quality of life and comfort. It addresses the physical, spiritual, and emotional needs of the patient and his or her family.
A facility for people who can no longer live on their own, but who don't need a high level of care. Assisted-living facilities provide assistance with medications, meals in a cafeteria or restaurant-like setting and housekeeping services. Nursing staff is located on site. Most facilities have social activities and provide transportation to doctors' appointments, shopping, etc.
