To obtain copies of your medical records for yourself or to send to someone else, you must complete and sign an authorization form.
You may complete the authorization form by either:
- Coming directly to the hospital campus.
- Downloading, printing and completing the online Authorization to Use or Disclose Health Information Form.

Photo Identification (ID) at Registration
Properly identifying patients is the first step in safety in any health care system. Improper identification can result in treatment errors and wrong-person procedures, as well as privacy violations, billing errors, insurance fraud and even identity fraud.
That's why UPMC requires all patients to show photo identification at registration. All patients age 18 or older are asked to present photo identification at registration.
Acceptable forms of ID include:
- Current driver's license
- Government-issued identification card
- Tribal identification card
- United States uniformed service identification card
- United States passport
- Certificate of Naturalization
- Employment card
- Student identification card
Please note that no one will be denied care if they don't have photo identification.
Patient Photos for Your Medical Record
For patient safety purposes, we will be taking patient photos using a webcam to be applied to our emergency department's electronic medical record. The photo will be stored in your file to identify you on your next visit. We’ll only need to take your picture once.
Copies of Birth Certificates
UPMC in central Pa. does not provide copies of birth certificates. Copies can only be issued by the PA Department of Health, Division of Vital Records. Their website, health.pa.gov/MyRecords/Certificates contains details on how to obtain a copy of a birth certificate.
