Our Home Visit Program is an innovative, comprehensive program that is available to promote a better quality of life for stay-at-home, chronically ill patients, and those who have significant barriers to obtaining consistent health care service.
This program is available to patients who are unable to leave their home. These reasons may include but are not limited to:
- Chronic illness.
- Need for assistive devices for mobility.
- Specific conditions where leaving the home is not possible.
- Use of specialized transportation or requiring physical assistance to leave the home.
Stay-at-home patients encounter a huge barrier to maintaining and improving their overall health. These patients generally lack primary care due to being unable to physically travel to the doctor's office unless an ambulance takes them to the hospital with an emergency. As a result, this scenario creates fluctuations in a patient's health status and can increase hospital admissions.
Because of our patient-centered approach, our team forms strong relationships with patients, caregivers, and family members. We respond to the medical needs of our patients, while addressing the social factors of health care.
Through regularly scheduled visits, we bring primary care to the home, which allows patients to:
- Keep up with their wellness visits.
- Maintain follow-up appointments.
- Acquire blood work and receive vaccinations in their home.
When basic primary care is maintained and the patient is monitored by professionals, they can more effectively manage their chronic illness, overall health, and remain stable.
