For our residents' convenience and comfort, we offer a variety of services at our facility, including:

  • Full-time nurse practitioner on staff to address immediate medical needs
  • State-of-the-art laboratory and X-ray facilities on campus
  • Specialty physician clinics on campus including ophthalmologist, dermatologist, orthopedics, and ear, nose and throat
  • Emergency Department on campus with 24-hour physician availability

Rehabilitation Services

Muncy PlaceOur full-service rehabilitation center offers quality on-site, personalized therapies for patients recovering from surgery, illness or injury. We have a fully-equipped therapy room to aid our certified therapists, physicians and nurses in coaching our residents back to their highest level of independence.

Our staff includes a contact physician and nurse practitioner for mid-level healthcare as necessary, as well as certified specialists for:

  • Physical therapy
  • Occupational therapy
  • Recreational therapy
  • Speech therapy
  • Respiratory therapy

Memory Care Unit (The Willows)

We provide individual attention and advanced care to residents with Alzheimer's disease and other forms of dementia in a beautiful and theraputic setting. We focus on the behavioral and physical needs of our patients, with private or semi-private rooms, soothing decor and personalized nutritional meal plans. Our RNs, LPNs, dieticians, social workers and nursing assistants have extensive knowledge of Alzheimer patient needs and help provide daily mind-sharpening activities, therapies and supervision.

Chronic Ventilator and Weaning Program

Our ventilator care program staffs a licensed respiratory therapist. Respiratory staff are equipped with the technology and techniques to treat ventilator/trach dependency in patients with respiratory failure, lung disease, cardiac disorders, neurological disorders, etc. The ventilator care program provides round-the-clock therapy, which is ordered by a pulmonologist and delivered by respiratory/nursing services to ensure the safety and well-being of our residents.

Hospice and Palliative Care

In addition to our primary services, we offer on-site palliative care and hospice services for patients with advanced chronic disease or a life-limiting illness. Our goal is to help residents face the physical, emotional and spiritual challenges of their condition and work with them and their families to provide comfort and support during this time. Our team can participate in discussion, actively monitor residents to manage pain and symptoms, and help patients obtain peace in their moments of trial.

Pennsylvania Orders for Life Sustaining Treatment (POLST)

POLST forms turn your wishes for medical treatment into actionable medical orders if you ever become unable to make those decisions yourself. The form stays with you as a reference for all your caregivers. Ideally, a patient should complete this during a discussion with loved ones and family members. It must be signed by your physician, certified nurse practitioner or physician assistant. The form addresses treatments such as CPR, comfort medical interventions, antibiotics and artificially administered hydration/nutrition.
