Discharge Planning Starts Early
Outpatient rehab services may take time to arrange. You and your family should start early when planning for discharge from inpatient rehab.
Our support staff will help you plan and arrange for the outpatient care you need. This will ensure a smooth transition from the hospital to home or another facility.
Your case manager or social worker can provide more details on your options.
Community Reentry
A smooth transition from the hospital to the community is vital.
Getting out and about can improve symptoms of depression and help you heal.
UPMC offers patients community reentry programs. These help you practice new skills in “real life" situations.
An outing may include lunch or shopping.
During the trip, the care team can identify problems and then address them during outpatient rehab treatment.
Transitional Rehabilitation at UPMC
This short-stay rehab is for people who no longer need inpatient care but aren't ready to go home.
Our doctors, specialty-trained therapists, and other transitional rehab staff provide:
- One-on-one, quality care focused on you and your needs.
- Continuity of treatment plans.
Outpatient Rehabilitation and Services at UPMC
Many people need rehab or other care after discharge.
Our case managers and social workers can help connect you with other providers if need be.
Outpatient rehab
Outpatient rehab is often the next step after leaving the hospital or skilled nursing. It may also be the best choice for people who don't need inpatient care.
Most insurance plans cover some outpatient rehab services.
Outpatient rehab may include:
- Physical therapy.
- Occupational therapy.
- Speech and language therapy.
Adult daycare for seniors
Adult daycare helps older adults stay in their homes rather than moving to a nursing or residential home. Insurance may or may not cover the cost of adult daycare.
Adult daycare takes place in a group setting and provides seniors with:
- A chance to make friends and socialize.
- Games and recreation.
- Basic personal care.
- Nutrition.
Some adult daycares may offer medical help such as:
- Physical therapy.
- Occupational therapy.
- Basic medical care.
- Nursing care.
- More intensive personal care.
Home health care
Some people returning home from a hospital or nursing home may need follow-up care.
UPMC at Home offers at-home health care services such as:
- Medication management.
- Physical therapy.
- Occupational therapy.
- Nursing visits.
These pre-scheduled visits are often short. Most health plans cover at-home care.
Long Term Care at UPMC
For people who aren't well enough to go home after inpatient rehab, long-term care may be an option.
Assisted living communities
Assisted living is the right choice if you're mostly independent.
These communities are often in a campus-like setting with a staff that can help with your:
- Medications.
- Diet and nutritional support.
- Personal care.
Insurance usually does not cover this type of care.
Skilled nursing homes
UPMC Skilled Nursing and Rehabilitation provides 24-hour nursing and other health care services. These homes are for people who can't live on their own.
A nursing home stay may be short-term until you're healthy enough to go home.
UPMC's nursing homes also provide physical and occupational therapy.
Health plans may cover all or part of nursing home costs.
Private duty nursing
Private duty nursing is available in a range of settings, including:
- Your home.
- At the hospital.
- Nursing homes.
- Assisted living communities.
Based on your needs, your private duty caretaker may be:
- Skilled professional or licensed nurses such as RNs or LPNs.
- Non-skilled providers such as certified or non-certified nursing assistants.
Private duty nursing services include:
- Medically necessary care.
- Personal care, such as help with bathing.
- Respite visit for caregivers.
- Companionship.
Care ranges from several hours a day to around-the-clock. Insurance usually does not cover this type of care.
